Discharge Procedures

Early during your stay, your health care team will talk to you about your after hospitalization care needs, anticipated discharge date, and how to receive your follow-up care.

If supplies are needed at time of discharge, the discharge nurse will review them with you. If medication is prescribed, it can be picked up at the outpatient pharmacy as you leave the hospital.

If you are on a special diet, a dietitian will counsel you and your family on how to follow your diet at home.

Post Discharge Call

You will be contacted within 24-48 hours after you have been discharged from inpatient care. We want to know how you are doing and invite you to ask any questions related to your care.

We will make sure you receive your discharge medication and the updated list of your medications.  We will also inform you of the follow-up appointments set for you.

Any comments you have about the care you received will help us to continue to meet our goal of providing quality health care to our veteran patients and family.

It is important that we have your correct phone number at time of discharge, especially if you will not be going to your own home.

Lost and Found

If you have lost any personal items during your stay, tell your health care team. You may also call (775)328-1405 or (775)326-5797 for assistance.
